Your medical records may be available electronically. For more information, please call (520) 694-7310 or visit the main lobby of Banner – University Medical Center Tucson, weekdays from 8 a.m. to 5 p.m.

When visiting the main lobby regarding medical records, please bring a photo ID with you. If the records you need are not available electronically, most requests can be filled within seven to 10 business days. However, factors such as record availability, size of the record and the volume of requests will affect response time.

There may be a charge for the cost of reproduction of medical records. To obtain the records, you need to complete a Records Release form below, which you can print, or it is available at the Release of Information window.

Request Your Medical Records Electronically

What is eDelivery?
HealthPort, our release of information vendor, can deliver copies of medical records in an electronic format. eDelivery provides an increased level of service by sending records electronically to a secure and environmentally friendly website.

How does it work?
Complete an authorization to release your medical records and then a request form for eDelivery (o pide la forma en español). You must have a valid email address.

Do I still have to pay for my records?
Yes, if you request records for personal use you get the first five pages free and will be charged 50 cents a page thereafter.

Who do I call if I have questions about the HealthPort website?
Call HealthPort at (800) 367-1500, #4.

Now that you filled out the paper work, what to expect:
After requesting an electronic copy of your medical records, HealthPort will, under agreement with this health-care provider, facilitate the release of the records based on your authorized request.

You will receive an email from HealthPort, at the email address provided, that will include detailed instructions on how to access electronic records via a secure web portal. Once you have received the email notification from HealthPort, the medical record will be available via the web portal for 30 days. If the record is not accessed during that timeframe, it will be deleted from the portal. If you need the record after that time, you must resubmit your request to the health-care facility.

To access your records electronically, your computer must meet or exceed certain requirements.

Payment regulations vary from state to state, therefore, depending on the location of the medical facility that you requested the records from, there may be a charge associated with this service. If that is the case, you may receive an invoice from HealthPort along with the medical record.

Request Your Medical Records by Mail

A top priority is to protect the security and privacy of patient information. All information contained within the patient medical record is considered confidential and is federally protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Medical records shall not be released without the written authorization of the patient, his/her legal representative, a subpoena or court order.

Tucson General Hospital
Occasionally, we receive requests for medical records from Tucson General Hospital, which closed in 2001. Our hospital does not have these medical records. If you were a patient at Tucson General Hospital and would like copies of medical records, please contact:
